At an affordable cost, our team of qualified and licensed physicians perform the medical assessments that the Ministry of Transportation requires drivers applying for a commercial license (A, B, C, D, E, and F) in addition to drivers applying for G and M licenses who do not meet the standards for peripheral vision to obtain.
Depending on the age of an individual, you may be required to submit a medical assessment form as frequently as every year to once every five (5) years. It is the patient’s responsibility to present themselves to a medical office when their medical exam is due.

Requirements for Commercial Drivers

Commercial drivers (Classes A, B, C, D, E and F) are required to submit a medical report when they apply for a licence. Commercial drivers are required to meet higher licensing standards because they drive longer hours, often in poor weather conditions, and they are subject to greater physical demands due to driving large heavy vehicles.

Medical Reports

Drivers with Class A, B, C, D, E and F licences must file additional medical reports as part of renewing their commercial driver’s licence
  • Every five years if they are under 46 years old
  • Every three years if they are between 46 and 64 years old
  • Annually if they are 65 years old and older
These requirements are in keeping with the recommendations of the Canadian Council of Motor Transport Administrators, which are the basis of the U.S./Canadian Reciprocal Agreement for commercial drivers crossing the Canada/ U.S. border. The ministry will send drivers’ medical forms 90 days before the due date. This will give you enough time to schedule an appointment with your physician or nurse practitioner and return the completed form by the due date. The form will be mailed to the address shown on your driver’s licence. If you move, you must update your address

On-site Full-service Medical Examination

Take note of the date you scheduled your exam on, as you will be required to have the exam completed at our centre. Make sure to be well-rested and well-nourished on the day of your examination! Note that follow-up tests and recommendations are also given to our clients when we see fit-- but don’t worry! You can rest assured that we will communicate any further requirements, as we push through the process.

Exam Result Submission

Once your test is complete, you can rest easy-- The form will be mailed to the address shown on your driver’s licence, which is usually mailed 5 to 10 days after your exam. We will also notify you once we submit the report. You can also check the status of your report online by CLICKING HERE.

Patients are required to pay the following fees for the medical exam.
Payment for the exam is due at the time of your appointment.
All parts of the exam are done on site during your visit, no additional travel required.

  • Urine testing
  • Vision testing to ensure drivers meet government set standards
  • Hearing test to meet MTO standards
  • Filling out of medical forms, including FORM – SR-LC-080 05-02
  • Forms outside of the MTO FORM SR-LC-080 05-02 (i.e. out of province, limo, taxi, etc.) can also be carried out at our facility, BUT at a premium rate
  • Conducting any other medical tests mandated by MTO
